Personal Document Checklist
If you have scheduled a Financial Planning meeting with Stitely & Karstetter, the following information will assist in giving your CPA and Financial Advisor a complete fiscal picture for purposes of helping you make financial planning decisions. Please bring copies of the following, if they apply to you and are readily available:
Business valuation reports
Real estate valuation reports
Most recent personal financial statement (if available)
Titling of assets – client, spouse, join (right of survivorship or tenants in common)
Individual account statements
Retirement account statements
Previous 2 years personal income tax returns (for new clients only)
Gift tax returns (for new clients only)
Wills and trust documents
Private foundation documents (if applicable)
Life insurance policies
Disability insurance policies
Deferred compensation plan information and statements (if applicable)
Split dollar plan information and statements (if applicable)
Stock option plan information (if applicable)
Restricted stock/incentive plan information and statements (if applicable)
